Medical Directives & Emergency Card
Life support, organ donation, DNR, medical POA. Printable wallet card for emergencies.
What is it?
Medical Directives let you record your wishes for medical care: life support, organ donation, DNR (Do Not Resuscitate), medical power of attorney. You can also create an Emergency Card—a printable wallet card with your blood type, allergies, emergency contacts, and key medical info.
First responders and family can use this in an emergency. It takes the guesswork out of critical decisions.
Why does it matter?
In an emergency, you might not be able to speak. Your family may not know your wishes. A medical directive and emergency card give them clarity.
Organ donation is a personal choice. Recording it ensures your wishes are known and respected.
How to use it
Fill in your preferences in the Medical Directives section. For the Emergency Card, add blood type, allergies, medications, and emergency contacts. Print it and keep it in your wallet.
Share a copy with your family or doctor if you're comfortable.
Tips
Update the card when your medications or contacts change.
Talk to your family about your wishes. They'll appreciate knowing.
